YOUTH MEDICAL RELEASE FORM-  To be a part of all the fun, you first have to have this form filled out by you and your parents.  This is your ticket to anything that is going on for a whole year.  When you turn this in, it will be kept in a book for future events so you won't have to worry about it later.  If you don't have one in the office yet, or you have a friend you want to bring, print  'em off and get them into the church office.  They are good till next school year, so you're set for the summer!
